Analyzer Accident Reporting Procedures for Onsite Employees (May 13, 2019)
- Call 9-1-1 if encountering life threatening medical emergency for immediate medical attention and get out of harm’s way.
- Report immediately to client’s immediate supervisor such as Caltrans structure representative or resident engineer. If unable to reach them, then call the Caltrans structures or district seniors. Then call, text or email Eng Yeong at (925) 325-1838 and eng.yeong@goanalyzer.com.
- For major injuries that require admission to hospital, use Kaiser card for workers comp or call Analyzer Workers Comp carrier, Employers Insurance Group, 24 hour hotline for advice or medical referral : (855) 365-6010. No preauthorization form is required.
- For other injuries that nonetheless require medical evaluation and attention, go to any clinic that accepts workers comp. The Analyzer Workers Comp carrier is Employers Insurance Group. - Provide information to Clients and help fill out and submit all required accident reporting paperwork following the Clients’ procedures.
- Employee is required to submit as soon as possible a short written report to the company detailing: 1. The day, time and place of the accident, and names of involved parties; 2. The circumstances and reasons for the accident; 3. The type of injuries sustained by all the parties involved. Use sketches if possible to illustrate and take photos to substantiate.
